WIMC Introduces New Chronic Care Management Program
At Wake Internal Medicine, we understand that as you get older, managing your health can become more challenging. Your health is our top priority and when you have a structured support system with your health care provider, we can provide the resources to help you remain as healthy as possible, for as long as possible. That is why we are introducing Chronic Care Management, a new service that will be offered to a select group of patients. This program will help us focus on assisting our patients that require a little extra care. This is a non face-to-face service that will allow your physician to have more oversight of your medical conditions between visits.
Who is eligible for Chronic Care Management?
Medicare and Medicare Advantage patients
Must have two or more chronic conditions that would merit additional coordination between office visits
Examples of chronic conditions:
Diabetes
Hypertension
COPD
Heart disease
Stroke
Obesity
Asthma
Kidney/Renal diseases
Dementia/Memory loss
Upon enrollment of this service, you will receive a customized care plan that is designed specifically for you by your physician. Then, our patient care coordinator will contact you periodically each month to see how you are doing, review your current medications, offer educational material, answer questions and communicate with you and your specialists. Our patient care coordinator is a dedicated person that you can rely on to help you and your family coordinate your health care needs.
