2016 Prescription Drug Formularies

2016 is right around the corner, bringing a new year and for some, new changes, like 2016 prescription drug formularies. A formulary is a list of prescription medications covered by your health insurance. 2016 prescription drug formularies vary by insurance company, including generic or preferred drugs (Tier 1 or 2), making the medication more cost-effective for you. Non-preferred or brand name drugs are usually in a higher tier and can cost more. Since formularies/tiers vary by insurance company and due to our large volume of patients, our staff will not be able to take the time to obtain a copy of your preferred drug list. It is the patient's responsibility to provide a copy of the preferred drug list (formulary) to the physician if a medication change is requested. Formularies change frequently, therefore it is important for you to stay up-to-date when your health insurance sends you the latest list. Your preferred and/or generic drug may change, requiring you to switch to another preferred or generic drug. With the impending new year, be sure to obtain an updated copy of your 2016 formulary. Please bring your formulary, insurance card, and prescription card with you to your next appointment. This will ensure that you and your provider can work together to find a medication that is the most cost-effective for you.
